Volunteer Application

I certify that the information provided on this application is true and complete to the best of my knowledge and agree that falsified information or significant omissions may disqualify me from further consideration for volunteering and may be considered justification for dismissal if discovered at a later date. I authorize persons, employers and organizations named in this application to provide the TMC Volunteers with any relevant information that may be required to arrive at any decision. I hereby release said persons and companies from any liability regarding the provision or uses of such information. The application process includes a personal interview, checking references, completing TB and drug screening, attending a general orientation and criminal background check. Upon acceptance, you will receive training to enable you to efficiently perform your duties as a volunteer. It is expected that volunteers will comply with the hospital and volunteer department policies and guidelines. Volunteer Services reserves the right to discontinue your volunteer services while at TMC. I further authorize my name and photograph to be used in Volunteer Services’ publications and other media.
