Medical Record Requests
The Health Information Management department maintains records of the care patients receive while they are at Texoma Medical Center. As required by state and federal law, these records are kept in strict confidence and are only released with proper authorization. We allow multiple ways for patients to obtain medical records and radiology studies.

Birth Certificate Information
To request a copy of a birth certificate for anyone born in Grayson County, contact one of the following locations:

Fees
Processing and copy fees may be applicable to your request. Costs will vary depending on the type of request. There are no fees associated with the patient portal, Health Records Online.
